Cervical Vertigo

Cervical vertigo refers to dizziness and imbalance following a substantial neck injury. The usual symptoms occur with movement of the neck and typically do not involve tinnitus (ringing of the ears) or hearing loss, although ear pain and neck pain are common.

There is some controversy as to the mechanism of cervical vertigo, but the main theories include 1) vascular compression, 2) alteration of sensory input to the vestibular system (a portion of the inner ear), 3) cervical cord compression, and 4) leak of CSF (Cerebral Spinal Fluid). For example, a whiplash injury may tear a cervical root sleeve, causing low CSF pressure and hearing symptoms.

Diagnosis of cervical vertigo can be an uncertain and frustrating process. There is no confirming diagnostic test available, so the diagnosis tends to be one of exclusion, i.e. a diagnosis reached by identifying hallmark symptoms and diagnostic results and eliminating reasonable alternatives.

Treatment plans often include physical therapy and medication, including muscle relaxants, pain medication, steroidal and non-steroidal anti-inflammatories, and anti-depressants. Surgical and external neck immobilization are sometimes employed in instances where more conservative measures fail to offer relief.
